Support plans: 

At CL Haven Services Ltd we put our young people's needs above everything else. Therefore, pre-service needs are assessed, and the support plan is robust, person centred and targeted to the young person’s individual circumstances prior to the start of service to enable and empower our young people to build on their strengths and achieve their long term goals and outcomes. 

Additionally our support staff will familiarise themselves with the young persons support plans, and the manager will ensure that all relevant information is carefully recorded and carried out. 

Young people we provide support for are:  

Young support leavers (up to 25 years olds where relevant) of any race, gender or sexuality. 

• 16 & 17yr olds placed under s20 of the Children Act 1989.

• Homeless Young People

• Unaccompanied Asylum-seeking Children/Young People 16-25 year’s

• Emergency Accommodation for Young People waiting to be placed to their matched placements.

• Young Offenders

Gender 

We support both male and female young people.

Nursing Support 

We do not provide nursing support.

Length of support Provided

The service will be provided for as long as is necessary and can be bespoke, appropriate support plans can be put in place with realistic time scales.
